  • Calquence/Imfinzi/Lynparza PAP – On MAF indication and patient’s monthly per capita household income is eligible for MAF subsidy

  • Do not have as-charged insurance (>90% coverage) or equivalent insurance which can cover medication costs.
  • Do not have employee healthcare benefits that can cover medication cost (e.g. Civil Service Card).
