Collection of program medicines at Hospital pharmacy
Yes, please bring along your Redemption Form/Card and show it to the hospital pharmacist when you are paying and collecting your chargeable medicines. Your complimentary program medicines will be issued to you at the same time.
Collection of program medicines at Unity pharmacy
No, not all Unity Pharmacy stores are participating in this program at this point in time. Once your application has been approved, an Access360 Program Consultant from Zuellig Pharma will contact you with the locations of the Unity Pharmacy that you may choose from.
Yes. You will need to call your Access360 Program Consultant from Zuellig Pharma beforehand at +65 3159 1015 and a new Program Card will be issued to you, reflecting the new Unity Pharmacy's location.
Once you’ve received your Doctor’s prescription and paid for your chargeable medicines at the hospital, you may then bring this Doctor’s Prescription, your Program Card and NRIC to the designated Unity Pharmacy store to collect your complimentary program medicines. This means that you can collect it on the same day (Hospital pharmacy first before Unity pharmacy).
You will need to bring along your Doctor's Prescription, Program Card and NRIC. If you are collecting on behalf of the patient, please bring a completed Authorization Form , Doctor's Prescription, Program Card and your NRIC.
